Here's a quick how-to for those of you who don't read English:

Download the full size image and print it on bright white paper. Then glue the printed image to the side of a recycled cereal box or other piece of thin cardboard. Then, cut around the outside of the shape. Don't forget to cut out the eye holes.

Now you have your mask! Decorate it with crayons, markers, glitter glue, feathers, or other craft supplies.

To wear: Poke holes in both sides of the mask, and tie a string so you can wear it. If you'd rather hold it up to your face, you can make a handle with an empty pwper towel roll, or wrapping paper tube.
